Video Hearing Requirements
If your case is scheduled for a video hearing, complete these steps before your hearing date:

How to Submit: Contact the clerk at jcfillmore@utcourts.gov for forms. Print, complete by hand, and email back to court.
Important: Failure to return forms may result in case continuation until received.

Frequently Asked Questions
When Do I Need to Appear at Court?
Timeline: You have 5 to 14 days from your citation to contact the court.
Action Required:
- Pay your ticket, or
- Schedule an appointment with the judge
Mandatory Appearance: Some charges require mandatory court appearance. You will be notified by mail with your arraignment date.
Contact Information: Notify the court if your address differs from the citation address.
Payment Option Available: If your charge does not require court appearance, you may:
- Pay by phone, online, mail, or at clerk’s window, or
- Request an appointment to see the judge
How Long Until My Citation Is in the System?
Generally, allow 5 business days for the court to receive citation information. You can contact the court anytime, but specific information about your case is only available after this timeframe.
Charges Not Requiring Appearance
You may:
- Schedule an appointment to contest the charge or make other requests
- Represent yourself or hire an attorney
- Pay the citation within 14 days by phone, mail, or at clerk’s window
Charges Requiring Appearance
- Arraignment Notice: Sent by mail with date and time
- Failure to Appear: A warrant may be issued
- Rescheduling: Submit written request at least 5 business days before hearing (attorney can request continuance on your behalf)
No Proof of Insurance Violation
This violation can be dismissed with verification from your insurance agency. Insurance card or benefits declaration is not sufficient.
Required Letter Information:
- On insurance agency letterhead
- Name of insured (with explanation if different from driver cited)
- Vehicle description (must match citation)
- Coverage effective date and time citation was issued
- Agent’s signature
- Citation or case number

- By fax to: (435) 743-4325 (must call (435) 743-6952 to confirm receipt)
If requirements cannot be met, call (435) 743-6952 to schedule appointment with judge.
If I Have a Warrant
Call the court at (435) 743-6952 for specific guidance.
Important: Warrant fee added to charges and driver license may be suspended.
If No Court Appearance Required:
- Pay warrant and receive driver license clearance
- Take clearance to Driver License Division for reinstatement
If Court Appearance Required:
- Clerk will require full bail (cash or card)
- Sign promise to appear before warrant is recalled

Everyone has the right to legal representation at any stage of the court process. If you cannot afford an attorney:
- Complete an affidavit with the court
- Be considered for a public defender (paid by city)
- Judge will review affidavit and determine qualification eligibility
- Court follows strict qualification guidelines
